Amount of weight regain after surgery can predict health risks

While on average study participants achieved maximum weight loss at two years post-surgery, when they reached that milestone the outcomes varied substantially with approximately 20 percent continuing to lose weight more than four years post-surgery

Measuring the percentage of weight regained following the maximum amount of weight lost after bariatric surgery can help predict a patient's risk of several serious health problems, according to a long-term, multi-centre study led by University of Pittsburgh Graduate School of Public Health researchers. The results also found that while on average study participants achieved maximum weight loss at two years post-surgery, when they reached that milestone the outcomes varied substantially with approximately 20 percent continuing to lose weight more than four years post-surgery.

Interestingly, no matter when maximum weight loss occurred, the rate of weight regained was highest in the first year following maximum weight loss. This indicates early detection and treatment of weight regain may be important.

"Our study can help providers identify the best way to calculate weight regain after bariatric surgery to identify more specifically the patients at risk for recurrence or new onset of health problems," said senior author, Dr Anita P Courcoulas, chief of minimally invasive bariatric surgery at UPMC. "It highlights the importance of longer-term, close follow-up to help maximise weight and health results following bariatric surgery."

The paper, ‘Comparison of the Performance of Common Measures of Weight Regain After Bariatric Surgery for Association With Clinical Outcomes’, published in JAMA, assessed whether there were approaches to measuring weight regain following bariatric surgery that are more predictive of clinical outcomes than others.

This prospective cohort study included 2,458 adults who underwent bariatric surgery at ten hospitals in six US cities between March 2006 and April 2009. The participants were enrolled in the National Institutes of Health-funded Longitudinal Assessment of Bariatric Surgery-2 (LABS-2), a prospective observational study of patients undergoing weight-loss surgery at one of 10 hospitals across the United States.

Assessments were conducted within 30 days’ pre-surgery, at six months’ post-surgery and then annually until January 2015. Of the 1,703 participants who underwent RYGB surgery, 1,406 (83%) were followed up and had their weight measured an average of 8.3 times over a period that averaged 6.6 years (excluding those who died or underwent surgical reversal). In addition, to weight regain, the researchers also looked at progression of diabetes, hyperlipidaemia and hypertension, and declines in physical and mental health–related quality of life and satisfaction with surgery.


Of the 1,406 participants who underwent RYGB surgery, the median age was 47 years and the median BMI46.3 prior to surgery. The majority were female (80.3%) and white (85.6%). The median percentage of maximum weight loss was 37.4% of pre-surgery weight and occurred a median of two years after RYGB surgery.

In addition to the percentage of weight lost that was regained, the team considered several other ways to characterise weight regain following maximum weight loss, including as a measure of BMI, as percentage of pre-surgery weight, as percentage of lowest weight and number of kilograms regained.

When each of the ways to measure weight regain was evaluated with respect to each of the health outcomes, the researchers found that the percentage of maximum weight lost had the strongest associations with the various health outcomes.

To understand how to calculate weight regain this way, they provided the following example: someone who lost 150 pounds after bariatric surgery, then regained 28 pounds, regained 19 percent of their maximum weight lost. This level of weight regain was associated with a 51 percent higher risk of diabetes progression and a 28 percent higher risk of decline in physical health-related quality of life, indicating to a clinician this level of weight regain may lead to the progression or development of various health problems.

In the first year following maximum weight loss, 10 percent of participants experienced diabetes progression, 26 percent high cholesterol and 46 percent high blood pressure. A clinically important decline in physical and mental health-related quality of life was experienced by 20 and 28 percent of participants, respectively, and 12 percent experienced a decline in their satisfaction with their weight-loss surgery.

Five years after maximum weight loss, the proportion of participants experiencing negative health outcomes grew to 35 percent for diabetes, 68 percent for high cholesterol, 72 percent for high blood pressure, 42 and 33 percent for decline in physical and mental health-related quality of life, respectively, and 28 percent for decline in satisfaction with surgery.

While the rate of weight regain decreased over time, the participants continued to regain weight throughout the LABS-2 follow-up, indicating that weight regain is something clinicians need to pay attention to over time, and not simply measure at one standard time post-surgery.

"Clinicians and patients want to know the extent of weight regain following bariatric surgery and how it may affect their health. However, the lack of a standard measure of weight gain and variable follow-up has led to inconsistent results across studies," said lead author, Dr Wendy C King, associate professor in Pitt Public Health's Department of Epidemiology. "Our study will help clinicians and patients understand the timeline, magnitude and impact of weight regain, as well as lead to further studies on how to best avoid and manage weight regain for better health outcomes."

The researchers noted that future work is needed to develop tools for patients and providers to more easily recognize and understand the impact of weight regain. To help with this effort, they created an online supplement with figures showing weight regain and decline in clinical outcomes over time following maximum weight loss.

King noted that five years after reaching maximum weight loss, participants maintained, on average, 73 percent of their maximum weight loss. "So, despite weight regain, in general patients are much healthier having had surgery," she added.
